Staff Resistance - Staff may not agree with the decision and resist the change, making it less effective.
The organisation may have policies in place that are restrictive and the decisions may need to be altered to suit policies.

Ability of managers, may not have the appropriate skills or initiative to make the best decisions and may be unable to cope with complex decisions or situations, resulting in a poor decision made.
Good quality information may not be available to help make informed decisions.
External Factors may limit or prevent managers from making decisions they planned.
