  • Primary Skin Lesions are abnormalities present from birth or develop later in life.
  • Macule is a flat, unelevated change in color, 1 mm - 1 cm in size.
  • Papule is a circumscribed, solid elevation of skin, less than 1 cm in size.
  • Plaque is a larger than 1 cm change in skin, including psoriasis and rubeola.
  • Nodule is an elevated, solid, hard mass deep in the dermis than papule, 0.5 - 2 cm in size.
  • Pustule is a vesicle or bulla filled with pus, including acne vulgaris and impetigo.
  • Cyst is an encapsulated, fluid-filled lesion, including sebaceous and epidermoid cysts.
  • Wheal is a reddened collection of enema fluid. examples are hives and mosquito bites

  • Bulla is a circumscribed, round or oval, thin mass with fluid or blood, larger than 0.5 cm
  • Atrophy is a dry, wrinkled skin from thinning or wasting of skin, loss of collagen & elastin.
  • Erosion is the wearing away of the epidermis, often seen as scratch marks or raptured epidermis.
  • Lichenification is a rough, thickened, hardened area from chronic irritation, often seen in chronic dermatitis.
  • Ulcer is a deep, irregularly shaped area of skin extending into the dermis or subcutaneous tissue, often seen as a pressure ulcer or stasis alcer.
  • Fissure is a linear crack with sharp edges, extending in the dermis, often seen as cracks at mouth corners or hands.
  • Scar is a flat, irregular area after a lesion or wound has healed, often seen as a healed surgical wound or injury, or healed ache.
  • Scales are shedding flakes of greasy, keratinized skin tissue, often seen in dry skin, dandruff, psoriasis, or eczema.
  • Crust is dry blood, serum or pus left when vesicles or pustules burst.
  • Scabs are large crusts that adhere to the skin surface, often seen in eczema, impetigo, herpes, or scabs.
