  • Stage one --> no voluntary or reflexive activity present in the involved extremity
  • Stage two --> movement only elicited by reflex or associated reaction
  • Stage three --> spasticity peaks, begin to see voluntary movement
  • UE flexion synergy patterns include elbow flexion, forearm supination, and wrist and finger flexion.
  • UE extension synergy patterns include shoulder adduction, shoulder internal rotation, and forearm pronation.
  • LE flexion synergy pattern includes hip flexion and external rotation, knee flexion, and ankle DF
  • LE extension synergy pattern includes hip adduction, knee extension, and ankle PF
  • Stage four --> begins to deviate from synergy patterns, specific movements are possible
  • Stage five --> speed and direction of movement improves
  • Stage six --> movement coordination near normal
  • Stage seven --> complete restoration of function
  • The most common medications for spasticity are baclofen and tizanidine.
