Torsion of the hydatid of Morgagni

  • Hydatid of Morgagni:
    • Torsion of this structure can lead to a similar presentation as testicular torsion
    • Hydatid of Morgagni = testicular appendage (appendix of testis)
    • Remnant of the Mullerian duct
    • Located on the superior pole of the testis
  • Torsion of these structures is more common in a younger age group than testicular torsion, and the scrotum is usually less erythematous with a normal lie of the testis.
  • The ‘blue dot’ sign may be present in the upper half of the hemiscrotum, which is the visible infarcted hydatid
  • Torsion of the hydatid of Morgagni is not a surgical emergency like testicular torsion. Managed conservatively.
    Surgical exploration still needed if any chance of testicular torsion
