Ejection Fraction Preserved Heart Failure

  • ejection fraction preserved heart failure:
    • ejection fraction is preserved
    • cardiac output is inadequate
    • this means there is a problem with filling - diastolic failure
    • e.g. a low EDVV of 90 instead of a normal 120, and a normal SV of 70
    • 70/90 = 77%, which is higher than what EF should be (70%)
  • ejection fraction preserved heart failure is associated with:
    • hypertrophy and/or fibrosis of a ventricle
    • as occurs in hypertension, or after an acute myocardial infarction or development of hypertrophic obstructive cardiomyopathy
    • hypertrophy and/or fibrosis - reduced ventricular chamber and compliance making filling difficult and limiting end diastolic ventricular volume
    • contractility is largely preserved so stroke volume is ok
    • as a % of EDVV, SV & EF is good, but because filling is poor, cardiac output will still be low
