• Emergent Admissions - this occur when a patient is initially seen in the emergency department and is subsequently admitted to the hospital.
• Elective Admissions - these are planned and scheduled in advance like surgeries
• The Client - Primary Source of Information
• Admission - official entry of the person in a healthcare facility
• Endorsement or clinical handover - this refers to the exchange of patient information during shift change
• Bedside Handover - to involve the patient and the family are significant others in the communication
• Same Facility - call the unit and go with the patient
• Another facility - duplicate records and make telephone call receiving staff giving them comprehensive report
• ISBAR - THIS ENHANCED SAFETY DURING THE TRANSFER OF CLINICAL INFORMATION IN HEALTHCARE SETTING
• ISBAR - identification, situation, background, assessment, recommendations
• OILS - MEMORY AID TO INTRAVENOUS ORDERS
• Orders - verify the accuracy of iv orders
• Infusions - check the infusion pumps and tubing
• Lines - inspect the iv lines for proper placement patency and securement
• Securement - evaluate the stability of the iv site
• Discharge - official departure of the patient from the healthcare setting
