    The client response is the same as the desired outcome

    Either a short-term outcome was achieved but the long-term goal was not, or the desired goal was incompletely attained
  • 5. Evaluation: “Did the plan work?” Whenever a healthcare provider intervenes or implement care,
    they must reassess or evaluate to ensure the desired outcome
    has been met.
     The possible patient outcomes are generally explained under
    three terms: Goal Met, Goal Partially Met or Goal Not Met
