    • a research instrument that consists of various types of questions intended to gather data from a targe population.
  • Types of Survey Questionnaires:
    1. Multiple Choice Questions

  • Multiple Choice Question
    • allows respondents to choose one or more options from a list of answers that the researcher provides.
    • helps produce easy-to-analyze data and provide mutually exclusive choices.
  • Dichotomous Questions
    • ones that only offer two possible answers, which are typically presented to survey takers in the following format.
    • also known as a binary response question.
  • Likert Scale
    • gives respondents a range of options that is used to gauge their opinions and feelings.
  • Demographic Questions
    • it subdivides the target population based on personal information.
  • Ranking Question

    • Binary Responses

  • Binary Responses
    • if a questions has only two possible response options, then it is a binary response option.
    • When both options are added, it is equal to 100%.
  • Single Select
    • if the participants are asked to pick one choice out of alternatives, it is a single-select response.
    • The total percentage selecting any category will add up to 100%.
  • Multiple Select
    • If participants are allowed to select "all that apply," the total number of responses will add up to more than 100%.
  • Net Promoter Questions
    • it is a rating scale, but is usually presented as a difference between two paired proportions.
