  • Lung Resection types:
    A) segmentectomy
    B) lobectomy
    C) pneumonectomy
  • Lobectomy:
    • Removal of entire lobe
    • Anastomosis (remaining area of where lung has been resected) stapled
    • Chest drain inserted to collect blood /air
    • Other parts of lung expand to fill space
    • Hemidiaphragm on affected side rises slightly
  • Surgical Variation:
    • Wedge Resection (only done when theres a very peripheral nodule or lesion that can be easily resected) / segmentectomy (removal of a segment)
    • Possibly larger air leak
    • more pleural exudate and bloodstained sputum
    • Usually for cancer or other isolated lesion
    • Sleeve resection - segmental bronchus gets resected too
    • Care with cough (end to end anastomosis)
    • segmentectomy = removal of a segment
    • lobectomy = removal of a lobe
    • pneumonectomy = removal of a lung
  • Pneumonectomy:
    • Removal of a whole lung (cancer)
    • Bronchial anastomosis stapled or sutured (main bronchus is closed)
    • Fluid collects in pleural space - solidifies and secures the mediastinum and remaining lung so it doesnt shift too much towards the empty side
    • Fluid monitored by chest x-ray
    • controlled by apical drain
    • Too much fluid
    • mediastinal shift
    • Over months fluid organises into fibrous tissue
