• reporting and recording of work related injuries, accidents and ill health is required by law
• the employer has a legal duty to report work-related injuries, diseases and dangerous incidents
• requires employers to keep written records of and report following incidents to hse
• work related accidents that cause death
• work related accidents that cause serious injury eg. loss or reduction of sight, serious burns, fractures, crush injuries causing internal organ damage, hypothermia or heat induced illness
• diagnosed cases of certain work related disease eg. carpal tunnel syndrome, tendonitis, exposure to biological agents, asbestosis, occupational dermatitis, occupational asthma, occupational cancer
• incidents that have potential to cause harm eg. collapse or equipment, explosions or fires, chemical spills and leaks, gas leaks, overflowing drains
