    A major component of the network information sources developed by the Department of Health (DOH) to enable it to better manage nationwide health service delivery activities

  • Objectives of FHSIS

    • To provide a standardized, facility-level data base which can be accessed for more in-depth studies
    • To minimize the recording and reporting burden at the service delivery level in order to allow more time for patient care and promotive activities

  • Individual Treatment Record (ITR)

    A basic patient consultation record or a bond paper which bares the following information: date, name, address of patient, height, weight, chief complaint and presenting signs and symptom, the diagnosis (if available), treatment and date of treatment of each family member who consulted in the health center
  • Target/Client Lists (TCL)

    Helps nurse/midwife plan and carry out patient care and service delivery, facilitates the monitoring and supervision of service delivery activities, reports services delivered, and provides a clinic-level data for population- based further studies/ research

    The midwife in the BHS accomplish this table on a monthly basis, and it can be the source of ten leading causes of morbidity for the municipality/city and help the nurse and MHO to get the monthly trend disease
  • Monthly Consolidation Table (MCT)

    An essential form in the FHSIS where the nurse at the RHU records the reported data per indicator by each PHN or midwife, and serves as the Output Table of the RHU as it already contains listing of BHS per indicator by each barangay health station
