PROCESS OF CONDUCTING FMEA:

    Cards (8)

    • Select the Process or System: Choose the process, system, or product to analyze using FMEA.
    • Identify Failure Modes: List all possible ways in which the chosen process or system could fail.
    • Determine Effects: Determine the consequences of each failure mode
    • Analyze Causes: Investigate the potential causes contributing to each failure mode.
    • Assign Severity, Occurrence, and Detection Ratings: Rate the severity, occurrence, and detection of each failure mode on a predefined scale
    • Calculate Risk Priority Number (RPN): Multiply the severity, occurrence, and detection ratings for each failure mode to obtain its RPN
    • Prioritize Mitigation Actions: Focus on addressing high-RPN failure modes by implementing corrective actions and preventive measures.
    • Implement and Monitor: Execute the identified actions, monitor their effectiveness, and continuously improve the process or system.
